ACANY

Created in 2018, Advance Care Alliance New York (ACANY) offers comprehensive care coordination to over 25,000 people with intellectual and developmental disabilities (IDD) throughout New York City, Long Island, and the lower Hudson Valley region. Funded by New York State, we have a large network of Care Managers who help people with IDD lead active, healthy, and fulfilling lives. As a pioneer in the field, we advocate for community integration and against institutionalization while developing innovative programs and approaches to supporting people with IDD.
