LIFEPlan

Formed in 2018, LIFEPlan is a New York State-funded Care Coordination Organization (CCO) dedicated to meeting the needs of people with intellectual and developmental disabilities (IDD). Highly trained, experienced Care Managers connect Members with services and supports throughout a 38-county region in upstate New York. We believe that every person should have the right to live the life they want to live, where they want to live so they can be happier, healthier and more fulfilled. LIFEPlan Care Managers actively seek out community-based agencies to provide advocacy and connect their Members to recreational, educational, and vocational opportunities.
